As detailed in your grant agreement, an Interim Report is due 5/15/26*.

 

Our staff looks forward to learning about the progress of your work under this Grant. Reports should be completed within this online grants platform. 

Both the interim report due now (and final grant report due at the end of your grant period on 6/1/27) should include the following, which must be submitted together:

Narrative Report (no more than 1,000 words summarizing your project progress and/or outcomes during the reporting period)

Signed Budget Sheet Update (documenting Project expenditures to date and verifying that expenses are within the approved scope of work)

Proof of Project Outcomes and MCHC acknowledgement (documents or copies of other intangible materials that are produced as a result of the Project, pictures of tangible products produced as a result of the Project, etc.)

Aside from a Signed Budget Sheet Update verifying that Grant expenditures are within the approved scope of work, additional financial documentation will not be required as part of the standard reporting process. However, by acceptance of this Grant, the Grantee certifies that it and any contractors or subcontractors of Grantee shall maintain accurate books, accounts, and records of all transactions relating to the receipt and expenditure of the Grant for the Project.
